HCC of Rural Missouri Live Well Community Health Center - Buckner Red Cross Pharmacy The Health Care Collaborative (HCC) of Rural Missouri, a 501 (c) (3) nonprofit, has expanded its health and wellness footprint to Buckner, Mo. HCC, which has Federally Qualified Health Centers (FQHCs) in Carrollton, Concordia and Waverly, is has also opened a Live Well Community Health Center in Buckner at 324 S. Hudson, 8 a.m. to 5 p.m. The facility offers primary care, dental and behavioral health care to area residents, and accepts all insurance including Medicare, Medicaid, private insurance and self-pay. Sliding fee scale is based on income for those who qualify. "Our goal is to take the patient-centered medical home focus that we incorporated at our other Live Well Community Health Centers and reinforce that comprehensive approach to care in Buckner," said HCC's Executive Director Toniann Richard. HCC and its Live Well Clinics have an established history of collaborating with rural, community-focused partners to provide programs and services to area residents. About Live Well Community Health Centers and Health Care Collaborative of Rural Missouri The Live Well Centers of Waverly and Concordia have been designated as Patient Centered Medical Homes (PCMHs) by the National Committee of Quality Assurance (NCQA). A patient centered medical home coordinates patient care to make accessing integrated services easy and seamless. In addition, it provides comprehensive care to patients in a manner they understand and is relevant to their needs. The HCC, a 501 (c) (3), develops, implements and supports countywide and sometimes regional programs that help create access to health education, awareness, prevention and treatment services.
